What we do

Age Well Indiana currently provides two HCBS waiver services. Each service is delivered according to the member's authorized service plan and applicable Indiana Medicaid requirements.

AC

Attendant Care

Support with qualifying activities of daily living and other authorized personal support needs.

  • Assistance with personal routines and daily activities that are authorized
  • Support that promotes safety, dignity, and independence
  • Services provided in the individual's home or other permitted community setting
Services must be included and authorized in the member's current service plan.
HC

Home & Community Assistance

Practical support that helps an individual maintain a safe home and participate in ordinary community life.

  • Assistance with authorized household and community-living tasks
  • Support that helps the person remain in a community setting
  • Flexible, person-centered help matched to the approved plan

Programs

Which Indiana programs can these services be part of?

Indiana's former Aged & Disabled Waiver was split in 2024 into the Health and Wellness Waiver for individuals under 60 and the Indiana PathWays for Aging Waiver for eligible individuals 60 and older.

HEALTH & WELLNESS • GENERALLY UNDER 60

Health & Wellness Waiver

An HCBS waiver that helps eligible individuals with disabilities receive supports in home and community settings as an alternative to institutional care.

  • Medicaid eligibility and HCBS waiver eligibility are required.
  • Individual must meet the applicable nursing-facility level-of-care criteria.
  • Services must be approved and included in the individual's service plan.

What we can provide: Attendant Care and Home & Community Assistance when authorized.

PATHWAYS • AGE 60+

Indiana PathWays for Aging

Indiana's managed long-term services and supports program for eligible Medicaid members age 60 and older, including those who need long-term services in a home or community setting.

  • Generally age 60 or older and eligible for full Medicaid coverage under the program.
  • Must meet the required nursing-facility level of care for waiver services.
  • The waiver benefit plan and service plan must be approved before covered waiver services begin.

What we can provide: Attendant Care and Home & Community Assistance when authorized.

Eligibility

Do I qualify?

Age Well Indiana does not decide Medicaid or waiver eligibility. State agencies, enrollment representatives, and care-management processes determine whether a person qualifies and which services are authorized.

Medicaid

You must have the applicable Indiana Medicaid coverage and meet the financial/eligibility rules for the program.

Level of Care

The state must determine that you meet the applicable level-of-care standard for the waiver.

Service Plan

Your approved plan must authorize the specific services and units you need.

Provider

Once authorized, you may choose an available approved provider such as Age Well Indiana.
