Generally for eligible individuals under age 60 with disabilities who meet Medicaid, HCBS, and nursing-facility level-of-care requirements.
Indiana PathWays for Aging
Generally for eligible individuals age 60 and older who have full Medicaid coverage through PathWays and meet the required nursing-facility level of care.
Important: Medicaid/waiver eligibility and service authorization are determined by Indiana FSSA/OMPP processes. Age Well Indiana does not determine eligibility.
